These certificates are paper certificates that are snail mailed to either you, or the recipient.  If you want us to send the certificate to the recipient, type their address on the line.  If you want us to send the certificate to you, so you can deliver it to the recipient, type your name and address into the line.  Use a hyphen to separate name, address, city/state.

These are single use certificates, meaning the full total of the certificate must be used in a single purchase.  Credit balances do not carry over on these certificates.  Certificates expire 1 year from the purchase date.
